The Worcester County Health Department is located in Snow Hill, Maryland on the beautiful Lower Eastern Shore of Maryland.
Our mission is to promote health, well being, and a safe environment for all of our residents, as well as those who visit our resort areas each season.
We accomplish this by assessing the community's needs, developing appropriate policy to promote health and well being, and providing, or assuring the provision, of needed quality health services. We currently have approx. 275 employees with a variety of educational backgrounds who specialize in such areas as: Environmental Science, Nursing, Social Work, Prevention and Health Promotion, Chemical Dependency Services, Administrative Services, and Mental Health.

The main purpose of this full time contractual position is to provide community based case management to adults and clientele with behavioral health disorders who are having difficultly meeting their basic needs. These basic needs most often include shelter, food, and access to entitlements. This position meets with the individuals in their home, shelter and or community and helps the individual gain access to resources such as stable housing, food stamps, ID cards, social security income, and alternative transportation options (based on eligibility.). This position also seeks to engage the client in behavioral health services and refer to other resources available as appropriate. Our agency believes in and supports a holistic methodology. This position requires a strong desire to help clients, understanding and patience in the approach with clients is required as well as being open to person centered techniques. This full time contractual position assists Worcester County children, adolescents and adults (who have a qualifying mental health illness). PRP is intended to teach life skills which would prevent the need for a higher level of care (hospitalization, incarceration and/or inpatient facilities) and ultimately teach the participating individuals how to succeed on their own. Work would include but is not limited to assessment, individualized goal planning, referrals, linkage and most commonly, skill building within the home and community. Services are provided individually and in group settings. A high level of coordination and communication is expected within the Behavioral Health unit. Team meetings often include the treating psychiatrist, therapist, supervisor(s) and at times, primary care services. Our agency believes in and supports a holistic methodology. The main purpose of this position is to provide direct services to clients of the Behavioral Health Program as part of a dynamic team, at a Joint Commission accredited organization. This position will work with diverse populations, including children, adolescents and adults in a work environment that offers flexibility and opportunities for innovative practice in a multi-disciplinary setting. This position is responsible for evaluating, assessing, treating and diagnosing individuals to determine necessary psychiatric treatment. This position will also conduct individual, group and family counseling, making determinations on appropriate community referrals, prescribing and monitoring medications, case coordination with other providers, maintaining appropriate clinical records in accordance with program policies and regulatory standards, treatment planning/implementation, crisis intervention and management when necessary. Must be able to work and travel to all sites of the Worcester County Health Department.
